2016 RIYLL SCORE REPORT FORM- Coach of HOME team must get a score sheet signed by the OPPOSING coach after each home game. Be sure to include any EJECTIONS as those must be reported to program Directors and RIYLL. Hang onto the form in case of any disputes. RIYLL does not need these and will only ask for them in case of a disputed score.

Adobe PDF file Player Zip Code Boundary Waiver Request *- The purpose of this form is to request an exception to the player's zip code boundaries as set forth by RIYLL. Player exception for playing outside the boundaries drawn up by RIYLL are limited to the rules established by RIYLL under the heading 'Playing Out of Zip Code'. This request is not an approval and will be reviewed by the Competition Committee. This form is to be completed by the requesting program’s President or Program Director ONLY.
